Application for Ballot by Mail

To get an Application for a Ballot by Mail:

  • Call or email the Hays County Elections Office to request an Application for a Ballot by Mail at 512-393-7310, elections@co.hays.tx.us.
  • Or download, complete, sign, and return Application to Vote by Mail (English)(Spanish)
  • Fill out, sign, and mail to the Hays County Elections Administrator: 712 S. Stagecoach Trail, Suite 1012, San Marcos, TX 78666
Choose a reason for Applying for a Ballot by Mail:

  • 65 Years of Age or Older
  • Disability (as defined in Texas Election Code 82.002(a), see instruction on reverse) By checking this box, “I affirm that I have a sickness or physical condition that prevents me from appearing at the polling place on election day without a likelihood of needing personal assistance or of injuring my health.”
  • Expected to give birth within three weeks before or after Election Day
  • Expected Absence from the County
  • Confined in Jail or Involuntary Civil Commitment (You may only apply for a ballot for one election and any resulting runoff)
Tips for Marking a Mail-In Ballot

If you need help reading, marking, or mailing the actual ballot, ask a trusted relative or friend for help. It’s not uncommon for someone from a political organization to offer to help with your ballot soon after you’ve received it. The Secretary of State's Office recommends you decline this kind of help for several reasons. If you allow your ballot to be mailed by someone you don’t know, it might not be mailed at all. If it’s delivered to the elections office by a common or contract carrier from the address of a candidate or a campaign’s headquarters, your ballot will be rejected.
Please note: Applications for Ballot by Mail are only available from the Elections Administrator by specific request. Political parties may legally send them out without a request.
